When quoting or submitting applications for Personal and Commercial Lines of Insurance we collect additional information such as your education and work experience, and other details of your personal life, such as your date of birth, the names and ages of your spouse or children, and your hobbies.

* Applications, organizers, or other documents that supply personal information

* Tax-related documents you provide that are required for processing tax returns, such as Forms W-2, 1099R, 1099-INT and 1099-DIV, and stock transactions.

Why is the information collected?

We only collect personal information that is necessary to file your business or personal tax return, to ensure we are properly evaluating the risk when quoting or submitting insurance policies and as well as when processing Vehicle registration transaction, sending money or paying your utility bills.

The information collected from you is primarily used to complete the transactions requested by you. In some instances, we my use such information to market to you. You may receive email ads, telemarketing calls, or mail offers from our office in regards to new services or promotions that we might be offering at a specific time.

We restrict access to personal information concerning you, except to our employees, insurance companies or Banks (in case you had requested a bank product to receive your income tax refund) who need such information in order to provide products or services to you.

Can you review or correct your personal information?

We allow our customer to review and request changes in the customer's own personal information. We will need your complete name, address, policy number (if applicable), daytime phone number and a copy of your driver’s license/state ID or may require other personal identification. We will respond to your request within thirty (30) days unless state law requires us to respond earlier. We will tell you with whom we have shared the information in the last two (2) years.
